Home Care Services at Caridad de San Antonio
MVCHS has provided Home Care Services in Mora County since 1992. We provide services for an average of 37 clients. We serve adults over 18 years of age who have been referred to our agency by the Adult Protective Service Division of New Mexico’s Children, Youth and Families Department. Eligible participants include those who have been abused, neglected or exploited; those who are have been determined to be at high risk for abuse, neglect or exploitation and those with physical or mental disabilities. We also serve adults who are in need of restorative and rehabilitative services to attain their highest level of functioning -- or to prevent inappropriate institutionalization
Services include assessment of client needs and the development of a service plan. According to the individualized plan, participants receive the following:
Personal Care: Personal Care involves providing hands-on or stand-by assistance for persons having difficulty performing activities of daily living. These services include: assistance with personal hygiene (bathing, dressing, grooming, toileting etc.); assistance with mobility to get from bed to chair, to wheelchair, bathroom, or eating area; meal preparation; grocery shopping, menu planning, assistance in eating; emotional support including the provision of socialization opportunities; assistance in interpreting physicians’ instructions, ensuring prescribed medications are taken; monitoring the health and physical status of the patient. Specialized care is offered to patients with severe disabilities, who are non-ambulatory, or who have had a recent illness or hospitalization. These services are provided through written instructions from the primary physician, home health nurse, public nurse, or physical therapist in accordance with NM laws and regulations. The services may include assisting the patient in implementing a professionally prescribed and monitored exercise regimen to increase mobility in the home; and/or assisting the patient with special devices such as braces or artificial limbs.
Home Management: Home management involves light-duty housekeeping services for persons having difficulty performing instrumental activities of daily living. We ensure a clean and safe home environment necessary for the patient to maintain residence in the least restrictive and safest setting to alleviate or prevent abuse, neglect or exploitation. Assistance may be given in the form of light house keeping, meal preparation, minor repairs to clothing, laundry; errands and shopping, transporting and accompanying persons to medical appointments, on errands and shopping trips, as well as money management and assistance planning/budgeting expenditures.
Environmental Safety/Chore Services: Environmental Safety and Chore Services include the provision of heavy-duty, more labor-intensive housekeeping services (i.e., hauling water, chopping wood, etc.) and special household maintenance (repairing furniture, installing screens, fixing safety rails, replacing fuses, locks, plugs, cleaning attics to remove fire/health hazards).
