Adult Day Services at Caridad de San Antonio
MVCHS provides on site adult day care services to adults over 18 years of age who have been referred to our agency by the Adult Protective Service Division of New Mexico’s Children, Youth and Families Department. Eligible participants include those who have been abused, neglected or exploited; those who are have been determined to be at high risk for abuse, neglect or exploitation and those with physical or mental disabilities. We also serve adults who are in need of restorative and rehabilitative services to attain their highest level of functioning -- or to prevent inappropriate institutionalization.
Services include assessment of client needs and the development of a meaningful service plan. According to the individualized plan, participants receive the following:
Psycho-social services: These include activities within the facility and through inclusion in community events that allow participants to develop satisfying interpersonal relationships, develop cultural enrichment and participate in recreational and educational areas of interest. Activities consist of coordination and/or provision of transportation to/from our Adult Day Care Center; providing small and large group activities for mental stimulation and social interaction, special events such as parties, holidays, etc. during which we encourage staff and family participation; providing opportunities for arts and crafts to encourage creativity, and in activities that will help develop cultural enrichment; offering opportunities to participate in activities and events in the Mora Valley; providing a meal and snacks each service day (within dietary restrictions of persons served); and offer an area for persons served to rest, to have private quiet time, or to visit with friends, family and staff.
Therapeutic/Rehabilitative Services: Through specially-trained and competent staff members, we provide direct assistance and skill-building in the performance of activities of daily living -- to improve the capacity for independent functioning. Therapeutic/Rehabilitative services include conducting an initial comprehensive health assessment and on-going monitoring of the health and physical status of the participant; providing assistance with activities of daily living (e.g., eating, personal hygiene, toileting); stimulating muscles and maintain range of motion through gentle exercise and guided movement (with physician’s approval and under proper supervision); and offering skill-building training to improve performance of instrumental activities of daily living. Health monitoring is provided by a staff RN and contracted therapists.
Family Support/Education/Consultation Services: Our staff is specially-trained to provide information and support to participants and their family members and caregivers, to help maintain the family unit and enhance the family’s overall health. Services include: Structured support sessions with the clients and their families and information sharing.
